Outside Pharmacy Waiver
Dear Client, We have received a request to fill a prescription for your pet from an outside pharmacy. Before we move forward, we want to make sure that you are aware of the following information. Hebron Animal Hospital guarantees that any prescription medication, product or food purchased from our office has been inspected and approved by the food and drug administration and has been manufactured and packaged in the United States according to government regulations. If your pet has a reaction to a product, food or medication purchased from our office, we can work with the manufacturer to cover the cost of treatment from adverse reactions and/or failure of the medication to prevent disease. Many companies only honor product guarantees when a veterinarian prescribes and sells the product. Purchases from unauthorized distributors do not qualify for the same guarantee. The majority of human pharmacists get little to no training in veterinary pharmacology and most pharmacists are not knowledgeable about animal physiology. This may lead to pharmacists suggesting changes to your written prescription, offering inaccurate substitutions, or they may describe a drug’s effects on humans, even though those side effects do not occur in animals etc. Your pet must maintain a current client patient relationship and receive their wellness exams on time to be eligible for any medications to be approved or prescribed. In some cases, blood work or other testing may be required periodically to monitor your pet’s health while on long term medications and we will inform you of this need in a timely manner. Please also be aware that If you have remaining refills available from a written prescription and decide you would like to have the re-mailing refills transferred to a different pharmacy, please contact that pharmacy. The doctors at HAH will not provide additional written scripts with refills remaining on the previous script. As always, the doctors and staff at Hebron Animal Hospital are committed to the health and safety of your pet. If you would still like to purchase your prescription from an outside pharmacy, please sign this document indicating that we have informed you of all the possible risks associated with purchasing your pets medications from an outside pharmacy. By signing this document, you agree to accept any and all responsibility financial or otherwise that may occur from this decision and you understand that Hebron Animal Hospital will be held harmless from any adverse reaction or complication resulting from the use of products or prescriptions of any kind purchased from sources outside the hospitals monitoring and control.
